JCPC Program and Court Counselor Feedback Tool:

Please use this page only AFTER you have communicated with the program staff members directly as a first alternative -- we do not condone complaints where folks may not have tried to resolve disputes/complaints/problems using informal and positive communications as a first choice. If you've tried to work things out with the staff and/or supervisor, and have not been able to improve the situation or resolve things, then this web page offers you a private place for your to register any concerns or complaints about services involving Cumberland County JCPC programs, the Council itself or any issues relative to program/staff interactions with the Court Counselors office ... please use it seriously and not as a general "gripe" page. You should only be entering information here if you want specific action and follow-up from the JCPC Coordinator, the Council and/or the Court Counselors.

You must provide your contact information so that we can know with whom to follow-up:

Name
Title
Organization
Street Address
Address (cont.)

Zip/Postal Code
Work Phone (best contact number)
E-mail

Enter today's date:

What is the nature of your concerns or feedback? Please be very specific. Give names, dates, specific staff members involved, and a brief but clear description of what happened and what you'd like to have the JCPC Coordinator or Council follow up on.

Select only one of the following (rate how serious you believe the problems to be) to help judge how critical or severe your situation is, and to point to the speed with which someone needs to respond:

Choose any from the following options what you may have already done regarding these issues or concerns (remember, this page is a 4th or 5th step in negotiating conflict or resolving interagency problems):

Spoken directly with the program staff about the problem
Spoken directly with the program's supervisor(s)
Spoken with your own supervisor(s) about the issues
Written anything to document the problem
Sent any written documentation to anyone
Filed a formal complaint or grievance with anyone

And finally, add any comments here as to what you think the solutions should include. Remember, these are just your thoughts and are not formal recommendations as to what another program or supervisor should do -- here we're asking for constructive feedback and input as to how you believe folks can solve the problems or issues:
